Step 1: Introduction

This guide has been prepared to assist students in using the Student Portal to complete the Form Four student SELFORM. The guide outlines the steps that a student should follow during the completion process.

Step 2: How to Log In?

Prepare your PREM number and your name exactly as they are registered in the SIS system. This information must match the records held in the SIS system. In your web browser, enter https://sis.tamisemi.go.tz

Step 3: Select account type

i. Click Create account (Student/Parent).

ii. Select Student, then Continue

Step 4: Confirm details and create a password

i. Enter the PREM Number and Middle Name exactly as they appear in the SIS system.

ii. Click CONFIRM DETAILS.

iii. Then, follow the system prompts to create your password. Keep it confidential.

iv. You will have successfully created your account.

Step 5: Log In to the System

i. In the login section, enter your PREM number.

ii. In the password field, enter the password you created.

iii. Then, click LOGIN.

If you are unable to create an account or log in to the system, contact the Head of School to verify your details in the SIS system.

B: KUJAZA NA KUTUMA SELFORM

Step 6: Open Selform

After logging into the system, you will see a menu displaying Dashboard, My details, Selection profile, Academics, and Selform. Click on Selform to open the selection form.

The form will open, showing the following sections:

i. Particulars – taarifa binafsi

ii. Choice – Machaguo (Pathways)

iii. Details – taarifa za ziada

iv. Declaration

v. Review

Step 6: Step One – Particulars (Personal Information)

This section displays the student’s information. Please review the details to ensure they are accurate. If there are any errors or omissions in the student’s information, the system allows you to correct the following fields:

i. Gender

ii. Date of birth

iii. Parent or guardian’s phone number

iv. Local authority/council of residence (Home Council)

v. Email address

vi. Disability status (if applicable)

If there are errors in any other information, please contact the Head of School for
further assistance.

Step 6: Step Two – Choices

This section allows the student to organize their academic choices by assigning them priorities from 1 to m5, where priority 1 represents the most important choice. Ensure that all five study options have been assigned a priority.

Step 6: Step Three – Details

This section allows the student to make detailed choices for each priority group by selecting their preferred specializations or courses, along with the schools or colleges that offer them.

These choices must be arranged in accordance with the priorities and requirements established within the system.

Step 6: Step Four – Declaration

Review and confirm the accuracy of the information provided for your selections, then accept the declaration by following the system’s instructions.

Step 6: Step Five – Review – Check and submit

Review the accuracy of the information and the arrangement of all the selections you have entered. If you are satisfied, click the button to submit your selections to the Head of School for review and
confirmation. Ensure that the status of the selections has changed from ‘Draft‘ to ‘Awaiting Verification‘.
